Gemstone at Creekside is a memory care community in Plano, Texas, dedicated to residents living with dementia, Alzheimer's, and other forms of memory loss. We believe caring for each resident starts with knowing the person as more than their diagnosis. Our compassionate caregivers take time to learn each resident's unique care needs, dietary requirements, and activity preferences so we can tailor care in the way that best suits them. Our experienced team provides personalized, hands-on support and specialized memory care designed to promote dignity, familiarity, engagement, and quality of life.
From our care team, med techs, and chef to our clinical director, sales director, and maintenance team, every member of our staff makes it a priority to give residents an environment that is safe, comfortable, and fun. Our team is onsite 24 hours a day, and residents enjoy three home-cooked meals daily, laundry and housekeeping services, activities, and more. To reduce confusion and stress, meals are served at consistent times each day with place settings provided, and assistance is managed for residents. Care is thoughtfully adjusted as needs evolve, so families can feel confident their loved one is supported with patience, compassion, and respect.
Safety is built into every part of the community. Keypad entries and a 24-hour security system are in place, along with specialized features such as a large circular walking path, wayfinding cues, and an enclosed garden and courtyard that let residents move about freely and securely.
Residents may choose a spacious private suite or one of our thoughtfully designed semi-private suites, which offer companionship while still maintaining comfortable personal space. For many residents, having a companion provides added social connection, familiarity, and reassurance throughout the day.
From personal care and medication support to dining, activities, meaningful engagement, and everyday routines, each part of the experience is designed around the resident.

Yes, This is your loved one’s home. The only requirement is that you signed your loved one out & request their daily medications if needed.
